What do you need for your appointment
Find out what you need to bring for your first or review appointment and associated consultation fees.
First appointment
Please bring the following to your first appointment with Professor Choong:
A referral letter – A referral letter from your GP or specialist is required to see Professor Choong to ensure that you are paid your Medicare rebate entitlement. GP referrals are usually valid for 12 months from the date of attendance and specialist referrals are valid for 3 months.
Imaging Requirements – Please bring all relevant X-rays and scans with you to your appointment. Professor Choong may send you for further scans or investigations if required.
Prior to your appointment:
- Please complete Patient Registration Forms and return prior.
- Please complete list of Medications and known Allergies.
Please bring to your appointment:
- Medicare card
- Private health insurance details
- CD or films
- Current referral
- List of medications
- List of known allergies.

Review appointment
Please bring the following to your review appointment with Professor Choong:
A referral letter – Central Melbourne Orthopaedics staff will inform you if you need a new referral from your GP or specialist.
Imaging Requirements – Please bring all relevant X-rays and scans with you to your appointment.
You may be asked to update your:
- Patient registration information – To ensure we have your up to date details
- Medical information – List of medications and known allergies as these may have changed since your last visit.
Please bring to your appointment:
- Medicare card
- Private health insurance details
- CD or films.

Fees
Please find Professor Choong’s consultation fees below.
Please ensure you have a valid referral from your GP or specialist prior to your scheduled appointment to ensure you are eligible for a Medicare Rebate. Provided an up-to-date referral has been provided at the time, we will submit the account directly to Medicare on your behalf for your rebate.
Consultation fees are not claimable from your private health insurance fund.

Your surgeon will determine the item number you will be charged for at the time of your consultation.
Accounts
Fees are payable in full on the day of consultation by Cash, Credit or EFTPOS cards. We do not accept AMEX or Diners cards.
You can conveniently pay by:
- Cash
- Eftpos
- Visa
- Mastercard
Provided an up-to-date referral has been provided at the time, we will submit the account directly to Medicare on your behalf for your rebate. Consultation fees are not claimable from your private health insurance fund.
